GIFTS

Employees and retirees who join the program are eligible to win multiple awards either as part of a group or as an individual.

1. Register. All employees who register for the WellFit for Life program will be given a "Measuring Tape" to measure your inches.
2. Finishers. Participants who complete the program according to the guidelines will receive a drawstring bag for toting gym clothes or other items and will be eligible to print a WellFit for Life Certificate of Completion online. Finishing the program means exercising at least one half (1/2) mile a day, four days a week, for at least six weeks of the 10 weeks program and logging these miles online at http://www.wellfit.teachershealthtrust.org.
3. Division Winners. Worksites that have the highest percentage of employees who enroll in & complete the program within their division will receive a t-shirt with the winning divisions imprinted.
   
   Division grouped as follows:
   1. (4) Elementary schools
   2. (3) Middle schools
   3. (2) High schools
   4. (1) Special school
   5. (1) Licensed department (Vision Services, Title I, Grants & Development, etc)
   6. (1) Rural school (Indian Springs, Boulder City, Laughlin, Sandy Valley, Moapa Valley & surrounding areas)
4. Lottery Winners. $40.00 Athletic Gift Certificate to Niketown (or either of the Nike Outlet stores) and a continental breakfast will be awarded to one work site of one division that has the highest percentage of participants enrolled in the program. The division winners will be selected through a lottery process by the Health Trust Wellness Department.
5. Mileage Winners. Athletic footwear from Nike will be awarded to one employee in each level who logs the highest number of miles and completes the program from each worksite division. Four individuals will be chosen in each category at the Elementary School level, three in each category at the Middle School level, two at the High School level, and one from the Special Schools, Rural Schools, and Specialty Departments. Gifts are awarded to participants according to the following levels of participation:
   • Novice: Beginners who do not exceed 75 miles upon completion (winners may only win once in this category)
   • Intermediate: Competitors do not exceed 300 miles upon completion
   • Advanced: Competitors do not exceed 600 miles upon completion
   • Extreme: Competitors who exceed 600-plus miles upon completion

Special Awards

At the end of the program, the Health Trust recognizes various WellFit for Life enrollees for their outstanding efforts. The Special Awards are as follows:

• Lifetime Achievement Award: A WellFit for Life participant diagnosed with a lifetime disease who completes the program despite complications with the disease will receive a $40.00 Nike gift card, a photo and interview for the Fall 2008 Health Traxx and a plaque which will be presented at the 2008 Health Festival. This individual will be chosen by lottery process in the event that more than one participant meeting the criteria completes the program.
• Heart Savers Award: Participants who lose 15 pounds or more will be awarded a beverage caddy and a water bottle, courtesy of Pueblo Medical Imaging (to be determined).
• Coordinators: Coordinators who have 10 or more individuals from their worksites who enroll in and complete the program will receive a gift card to Sweet Tomatoes. Three (3) coordinators with 10 or more individuals from their worksites who enroll in and complete the program will receive $40.00 gift cards to Nike, Inc. These three (3) coordinators will be selected through a lottery process.
• The Best Weigh To Go Team: Three (3) teams who achieve the highest percentage of Team body weight loss will be awarded the following:
  • Grand Prize: $1,000 and before and after photos and interview in the 2008 Fall issue of Health Traxx
  • Second Place: $500 and before and after photos in the 2008 Fall issue of Health Traxx
  • Third Place: $250 and before and after photos in the 2008 Fall issue of Health Traxx
