Overview
The Teachers Health Trust has organized the 2009 Back–to–School Immunization Workshop to provide a convenient date and location for school–aged dependents of our participants to receive required vaccinations prior to commencement of the 2009—2010 school year.
This workshop is FREE TO ALL ELIGIBILE Diamond and Platinum dependents.
Eligibility
The 2009 Back–to–School Immunization Workshop is open to all Teachers Health Trust participants with eligible school–aged dependents currently covered under the Diamond or Platinum Plans in need of back–to–school immunizations. Participants should be sure to mark their calendars for the 2009 Back–to–School Immunization Workshop. The Teachers Health Trust has partnered with Hope Pediatrics Center to host this important workshop for all eligible dependents.
The workshop will be held at the Teachers Health Trust on Tuesday, August 11, 2009 from 10:00 a.m. - 2 p.m Space is limited so participants must RSVP to attend.
To attend the workshop:
- CLICK HERE to RSVP for the workshop. Space is limited and the Trust expects a large showing for the workshop. For this reason, all participants must RSVP for the event. All participants who RSVP will receive a confirmation call from the Teachers Health Trust. At this time, participants will be provided a time window for arrival to the workshop.
Please Note: The last day to RSVP for this workshop was Wednesday, August 05, 2009.
- CLICK HERE to complete the Malpractice and Consent Form. This form must be completed prior to arrival at the 2009 Back–to–School in order for the Teachers Health Trust and Hope Pediatrics Center to administer vaccinations to your dependent(s).

Fax a copy of your dependent(s) immunization records to Teachers Health Trust at (702) 794–2093 prior to the date of the workshop. Please include cover sheet with the following:
- Subject of 2009 Back–to–School Immunization Workshop
- Attention: Wellness Division
On the day of workshop:
- You must have your Teachers Health Trust card.
- Parents are strongly encouraged to bring child immunizations records.
- Be aware that participants will be served on first come, first served basis.
